The IVF Process is an attempt to conceive beyond natural means and is performed in accordance with the Health Act of the Republic of Indonesia No.36 Th 2009 Chapter 7 Article 127 Paragraph (1a): The effort of pregnancy outside the natural way can only be done by a legal partner with the following provisions: Fertilization of sperm and eggs of husband and wife are instilled in the womb of the wife from which the eggs come from.
 

The use of sperm donors, eggs and embryos and Surrogate Mother is prohibited in Indonesia.


NATURAL PREGNANCY PROCESS

  IVF PROCESS

The process of fertilization occurs in the mother's oviduct   The fertilization process is done in the laboratory (In Vitro Fertilization / IVF). After developing into an embryo then put into the mother's womb in the hope of developing into a fetus.

THERE ARE 2 TYPES OF TREATING PROCESS IN IVF

 
Husbands who have normal sperm counts then fertilization is done by pouring sperm that has been through the washing process into the culture media cup containing the egg.   Husband who has a very small number of sperm cells then his fertilization is done by injecting sperm cells into the egg.

 

PHASE IN IVF PROCESS

1. SELECTION STAGE

Laboratory examination: HIV, Hepatitis B & C in husband and wife. Besides that, there is also a complete blood examination of wife and sperm examination of husband. From the results of the examination of the new sperm can be determined the type of IVF process to be done (Conventional or ICSI). This should be done one week before the next menstrual cycle is expected, so prospective participants are expected to be absolutely sure to do the IVF program.

2. PHASE OF TREATMENT

On the 3rd day of menstruation, Vaginal Ultrasound (Trans Vaginal Sonography / TVS) examination and Hormonal Laboratory examination (E2, LH, FSH & Prolactin) are performed to determine the protocol of clearance and dose of egg stimulant drug to be administered.

The IVF Process is highly individualistic and there are several types of treatment protocols, depending on the individual's individual circumstances. Type of treatment protocol used in Prima Medika Hospital IVF Clinic:

  1. Short Protocol: starting on the 3rd day of the menstrual cycle
  2. Protocol Length: starting on the 22nd day of the menstrual cycle.

VAGINA USG EXAMINATION:

To measure the thickness of the inner lining of the uterus and calculate the Antral Folikel (egg bag). On the 3rd day of menstruation the thickness of the inner lining of the uterus is about 3 mm, the diameter of the Antral Follicle is about 2-8 mm in varying amounts. The number of follicles less than 5 tends not to respond well to treatment and will have too little undercooked eggs when taking.

   

EXAMINATION OF HORMONAL LABORATORY (E2, LH, FSH & PROLACTIN):

E2: E2 levels can be used to estimate the likelihood of successful pregnancy in the IVF process. On the 3rd day of menstruation the normal level of E2 is about 50 pg / mL or lower. E2 levels above 75 pg / ml will give a very low response to treatment. In normal cycles, initially low E2 levels will gradually increase until the follicle matures.

FSH: In general, 3 to 10 mIU / ml FSH levels on the 3rd day of menstruation showed good egg quality and high success rate, while FSH levels in the range of 12-15 mIU / ml showed lower egg quality and also low success. FSH levels over 20 mIU / ml are very low success rates.

LH: On the 3rd day of menstruation, the normal value of LH is between 3 and 10 mIU / ml. LH and FSH levels are more or less the same, In Polycystic Ovary (PCO) LH is higher than FSH.

Prolactin: Normal prolactin level less than 20 ng / ml. Prolactin enhancement may impair the thickening of the inner lining of the uterus.

TREATMENT :

Injection Stimulation Eggs to get a lot of eggs

It takes a lot of eggs because:

• Not all eggs are fertilized by sperm

• Not all fertilized eggs will develop well

 
On Natural Processes     In IVF Process

Stimulation Injections Eggs are done under the skin around the navel every day for ± 8 - 10 days. Monitoring is done 3 times by checking the hormones E2 and ultrasound vagina to know the success of treatment.

E2 MONITORING:

Monitor elevated E2 levels during treatment. If E2 levels rise too high (more than 3000 pg / ml) or if E2 levels do not increase the treatment is not continued (cancel)

Vaginal Ultrasound Monitoring:

1. Measuring the thickness of the inner lining of the uterus:

During treatment the inner lining of the uterus will thicken about 1 mm each day. The inner lining of the uterus should reach a thickness of 10 to 12 mm.

    

2. Counting and measuring the egg bag:

The number and diameter of the egg bag indicates the success of the treatment. The average egg bag grows About 2 millimeters a day. When it has reached about 18 - 20 millimeters given injection broke egg before Eggs are taken.

   

Treatment is considered successful when the results of monitoring of hormones E2 and TVS obtained:

  - E2 hormone greater than 600 pg / ml (200 - 250 / follicle)

  - Eggs mature over 3 with a diameter of 18 mm

  - The thickness of the inner lining of the uterus is more than 10 mm

 After treatment successfully given injections egg maturation.

Treatment is discontinued when the egg bag does not develop.

3. PHARMACEUTICAL TREATMENT STAGE

Performed between 34 -36 hours after injection of egg maturation. Taking eggs is done through the vagina with general anesthesia so it must fast 6 hours earlier.


  

Shortly before egg collection, the husband must remove the sperm by masturbating in the room that has been prepared in the clinic. In the process of masturbation is not allowed to use lubricant (soap or baby oil) because it can inhibit the process of fertilization. If there is no sperm cell in semen because of sperm blockage or vasectomy, sperm cell taking is done by Percutaneous Epidydimal Sperm Aspiration/PESA, ie inserting a small needle into the sperm shelter above the testicle using general anesthesia . If no sperm cells are found in the place then taken on the test using a small needle. The action is done by a Urologist Specialist.

 

FERTILIZATION PROCESS :

4. EMBRIO CULTIVATING STAGE

This process is a simple process so there is no special preparation to be done because there is no need for anesthesia. The mother usually does not suffer too much pain. The embryo is inserted into the uterus with special tool (catheter) being monitored with ULTRASOUND of the abdomen. The bladder must be fully charged (mother should hold the pee). Embryo cultivation carried out between day 2 to 5 after the egg Retrieval depends on the quality of the embryo. When embryos obtained in great numbers, to a maximum of just 4 embryos to be implanted. The rest would be frozen for safekeeping. When the process of Tube failure then the Baby can be repeated using embryos that had been frozen. The cultivation of the frozen embryos that have been thawed can be done after 3 menstrual cycles after the Baby Tube failure. Planting can be done with a natural Cycle or the cycle of the giving hormones.

5. STAGE OF CONCERNING HORMONE CONTENT

Content booster injection given on day 5, 8 and 10 after the egg Retrieval.

6. THE STAGE OF THE PREGNANCY TEST

The 15th day after the egg Retrieval is done the examination of blood (βhCG).
